Audiovisual Equipment Request Form

  • All events using a meeting space on campus must be approved through the Office of Information Technology before an audiovisual equipment request form is completed.
  • Please submit all requests at least one (1) week prior to event. Emergency situations will try to be accommodated, but due to the large number of events, we need ample time to plan our daily setup schedule.
  • Be as descriptive as possible when requesting your equipment.  Use the “Additional Information” section to describe all specific details.
