Getting your I-20.
Upon receipt of your acceptance letter from the OIP, and before we can issue your I-20 Form for visa purposes, you must do the following:
1. Admission Fee.
Pay a $50.00 non-refundable Admission Fee, (Link Credit Card) and send it to: firstname.lastname@example.org
2. Health Record Examination Form.
Have your doctor, or medical care provider complete the Health Record Examination Form (Link) and mail the original to the OIP. The form must be signed, and it must show your vaccinations and medical history.
3. International Student Residential Hall Agreement Form
Fill out the International Student Residential Hall Agreement form (link) and send it to: email@example.com
4. Refundable Enrollment Fee.*
Pay the $3,500.00 Refundable Enrollment fee ( link). This fee will be applied toward your tuition balance.
You will receive your I-20 Form within two weeks of receipt of all above items.
