ESSAY

The essay should be at least one page, but no more than two pages. Please include your intended major, career goals and tell how you will benefit from the scholarship.

Include your name, social security number and school on the upper left-hand corner of each page.

VOLUNTEER LEADERSHIP AWARDS AND ACTIVITIES

List all special awards, honors, extracurricular activities and leadership positions/offices held. You may use an extra sheet if necessary.
