STEP 1:

Agency requests CEU Application Packet from School of Continuing Education.

STEP 2:

Agency prepares and submits Application Packet to the School of Continuing Education a minimum of 30 (thirty) days prior to event.

NOTE: (1) Each application should be accompanied by the appropriate application fee. (2) Each CEU event must have a minimum number of 10 (ten) participants.

STEP 3:

The CEU Coordinator reviews the CEU Program Application and provides a Notification of Approval or Notification of Denial within 15 (fifteen) business days of receipt. Resubmission of application will be encouraged of all denied applications. Upon approval, the CEU Coordinator provides sample electronic documents of the Program Evaluation Form, Verification of Attendance Form, Participation Master Roster, and CEU Balance Payment Form.

STEP 4:

Upon approval, the CEU Coordinator establishes a course code registered with the campus permanent database for the CEU Program and publishes listing of the program in School of Continuing Education materials and on website. (Eg. CE 500 LA 60 07/3S Broadband Communications)

STEP 5:

Agency is responsible for conducting marketing, registration, instruction, and evaluation in accordance with the approved CEU Program Application Form. Agency is responsible for all aspects of the instructional program, including scheduling appropriate facilities, providing accommodations for persons with disabilities, ensuring accommodation of divergent learning styles, etc. Upon request, the School of Continuing Education may be contracted to provide support services, including, but not limited to registration, meeting space rental, food/beverage service, and promotional assistance.

STEP 6:

Within 5 (five) business days after the program End Date, the Agency must provide the School of Continuing Education final payment of $10.00 CEU Processing Fee per participant plus a Participant Master Roster listing all required information for all participants.

NOTE: All forms and payments must be submitted in one complete package per event.

STEP 7:

Within 30 (thirty) business days of receipt of the final payment and the Participation Roster, the School of Continuing Education mails CEU transcript to successful participants.

STEP 8:

If the Agency elects to offer the same exact instructional program in the future updated CEU Program Approval Form and Application Fee must be submitted at least 15 (fifteen) business days prior to offering the program.
